On Wednesday May 22, 2019, a woman was struck by a vehicle as she attempted to cross The Queensway at a pedestrian crossover immediately east of Milton Street. She activated the pedestrian signal light, pointed, and began to cross the street when a vehicle proceeded through the crosswalk and hit her. According to the City of Toronto's road classification system, The Queensway is classified as a 'Major Arterial' roadway.

Within the road section between The West Mall and Riverside Drive, The Queensway has a varying lane cross sections and a varying speed limit between 50 km/h and 60 km/h. A July 2017 Staff Report investigating the safety of the pedestrian crossover at two intersections along The Queensway notes excessive vehicle operating speeds on the street. This motion requests Transportation Services to conduct a study of the appropriateness of current speed limits on The Queensway between The West Mall and Riverside Drive.
