The Bid Award Panel adopted the following: 1. Bid Award Panel grant authority to award the contract in accordance with the Contract Details set out in the report (July 22, 2021) from the Chief Procurement Officer: Solicitation Number: Request for Quotation, Number 2957734599 Description: For Just-in-time Quantitative Fit Testing (QNFT) and Training on N95 Respirators for Toronto Public Health employees, if and when required at various locations within the City for a period of three (3) years, with the option to renew the contract for two (2) additional separate one (1) year periods.

Prior to the renewal of the contract, Purchasing and Materials Management Division will perform a market analysis to verify the current market conditions as compared to the proposed increase by the vendor and that the Stores Division will monitor vendor performance during the first term of the contract. Should the option(s) be exercised, then the Director of Toronto Public Health Division will request the Director of Purchasing and Materials Management to process the renewals under the same terms and conditions. Recommended Supplier: Levitt-Safety Ltd. Contract Award Value: $337,135 net of all applicable taxes and charges $380,963 including HST and all applicable charges $343,069 net of HST recoveries Contract is expected to start on date of award and end on July 31, 2024.
